Transport

For those patients with a medical need, transport can be arranged to hospital by the surgery. Please give as much notice as possible when phoning to request transport and provide as much information as you can. If you are phoning to request transport please try to ring late morning - tel: 01527 872008. If transport is required to the Queen Elizabeth or Selly Oak Hospitals patients are required to arrange this themselves by telephoning 0121 627 2098 - lines available Monday to Friday 8.00am to 6.00pm.

Carers

If you are the main carer for someone in their own home please let us know by completing a carer's information form at reception.

Written Correspondence

Your NHS number is a unique identification for you, so by using it in correspondence between different parts of the NHS the risk is mis-identifying a particular patient is reduced. We are trying to include your NHS number in all correspondence. Your NHS number can be found on your medical card.

Mobile Phones

Please switch mobile phones off while at the surgery.

Food and Drink

Please do not bring food and drink into the surgery.

Abusive/Violent Patients

The practice will not accept abusive or violent behaviour at the surgery under any circumstances. Patients will be removed from our list and legal action may be taken against persons acting violently.
